2026-2027 Athletic Physical Information

HCISD Athletes & Parents,

It is EXTREMELY important to read through the information below in order to appropriately submit the necessary requirements for athletic participation in the 2026- 2027 school year. All forms should be submitted electronically. All students currently enrolled in a HCISD school who are entering grades 7-12 should follow the instructions below using their student ID number to register (numbers only). **See notes at the bottom of this page for new students who are being enrolled to the district during the Summer**

We highly encourage all parents to register for a RankOne account while submitting these requirements in order to keep track of the forms that have been submitted. If you need to download a copy of any of the documentation submitted, you will also need to create an account through RankOne.

* Medical History
* UIL Forms Signature Page
* Emergency Contact Form

*Medical History, UIL Forms Signature Page, Emergency Contract Form can be submitted before submitting the UIL physical form. Some forms must be reviewed and will not be approved automatically, please allow time for us to review these forms and approve them. The training room is closed for the Summer, but we do check on the forms periodically to approve or deny them.

THESE FORMS MUST BE SUBMITTED ON RANKONE PRIOR TO PARTICIPATION IN ANY PRACTICE, SCRIMMAGE, PERFORMANCE OR CONTEST BEFORE, DURING OR AFTER SCHOOL.

**Students New to HCISD in the 26-27 school year**

The school district will rollover and automatically add new students into the system toward the end of July, so it is unlikely that new students who have enrolled in the district for the upcoming academic year, will show up when you attempt to submit the forms on RankOne. If you have the completed physical, please submit it via email to [email protected] PLEASE INDICATE THAT THE PHYSICAL IS FOR A NEW STUDENT in your email.
